Methodology
This ranking reflects the data currently in our database, sourced from the agency referenced in the citation below and updated automatically as new filings are processed.
Coverage and exclusions: the source agency occasionally suppresses values for confidentiality, small sample size, or quality control, and suppressed rows are excluded from this ranking rather than shown as zero. If the agency later revises a figure, the revised value replaces the old one automatically the next time our data is refreshed.
Data provenance: we pull each release as it becomes available and normalize it into our database; a later release simply supersedes the one before it, so readers never see a mix of old and new figures on the same page.
Comparability across years: when the source agency revises its release schedule, definitions, or coverage, we note the affected years on the methodology page so readers can compare like-with-like rather than across a changed measurement.

What this analysis cannot tell us
Fatality counts are aggregated from Form 300A summary boxes filed under OSHA's Injury Tracking Application program; the counts cover establishment-level summaries submitted for each reporting year. Single-establishment fatalities can be reported across multiple years if the case spans a reporting boundary, though OSHA recordkeeping guidance is to record the incident in the year of occurrence. Large multi-establishment employers (national chains, multi-plant manufacturers) accumulate fatalities across all reporting establishments; per-establishment counts are typically much lower. Employer-name normalization in the PlainSafetyScore dataset attempts to consolidate establishment-level submissions under a parent employer where the relationship is encoded, establishments filing under subsidiary names that PlainSafetyScore has not yet linked will appear as separate employers. Industry_avg_tcr is the average TCR for the employer's primary NAICS industry; comparison to latest_tcr is a useful relative-risk indicator but does not control for establishment size, work-mix differences, or reporting completeness. Safety_grade letters (A through F) are PlainSafetyScore editorial assignments using documented composite-score cutoffs combining TCR, DART, and fatality signals, they are not OSHA-published values. This page reports figures as recorded in the public OSHA Form 300A dataset; it is not advice on employment, contracting, or occupational risk for any specific worker, role, or facility.
